Local workflow issues

Bugs found while working on #1230

• ./generate_guideline_templates.py doesn't generate the main title (lines 1 and 2 in the PR)
• ./generate_guideline_templates.py doesn't add default bibliographic entries
• README / contributing docs don't mention --serve option for make.py

And while not exactly bugs, the following aren't exactly helpful for that workflow:

• We still need to think about Reference for Guidelines *not* related to FLS #583
• Can we easily check code examples for correctness locally?
• Guidance on the fields is only (afaik) in the new issue template :-D
